Acarbose description
Indication:
Acarbose is prescribed to help lower blood sugar levels that is an alpha-glucoside inhibitor obtained from the fermentation of process of microorganism which delays the digestion of ingested carbohydrates. It does not enhance insulin secretion and can be used alone to treat type II diabetes. It's effect are additive to those of the sulfonylurea in controlling blood glucose.
Mechanism of action:
Acarbose competitively and reversibly inhibits pancreatic á-amylase and intestinal brush border á-glucosidases, resulting in retardation of glucose absorption from hydrolysed complex carbohydrates and reduction of blood-glucose concentrations.
Dosage and Administration:
Initially it is given at 25mg daily. Dose is adjusted at 4 to 8 wkly intervals based on tolerance and 1 hr postprandial glucose levels. Give three times at the start of each meal.
For patients with severe kidney dysfunction or liver disease smaller doses may be given. It should not be given to patient with certain medical condition such as inflammatory bowel disease, intestinal obstruction, and chronic intestinal disease cause it interferes with digestion and absorption.
Drug Interactions:
Possible decrease in digoxin levels if combined, monitor patients closely if this combination is used. Decreased effects of acarbose if taken with digestive enzymes or charcoal; avoid these combination.
In combination with sulfonylurea, blood glucose may be much lower; monitor closely and adjust dosages of each drug accordingly.
Pregnancy:
It is set as a Category B by the US Food and Drug Administration which indicates that no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect.
Nursing Mothers:
It has found out that acarbose is excreted in milk by lactating animals. No human studies was conducted. Breastfeeding mother should talk to their doctor first before taking the drug.
Interaction with Alcohol:
As to its interaction with alcohol,it does increase blood sugar. It would be better to seek medical advice.
Adverse/Side effects:
Certain side effects may occur such as abdominal pain, flatulence, and bloating.Report unusual bleeding or bruising( symptoms of decrease hematocrit) and severe abdominal pain because its rare side effect can affect liver enzymes,calcium, or Vitamin B6 levels.
Storage:
This drug should be stored in a tightly close container, away from heat and moisture, and stored at room temperature. Should be placed out of reach of children.
